If you have had a back surgery, hip or knee replacement, or fractures, or if you’re living with joint disease, sports injuries or other orthopedic problems, we can help you get back on your feet.

CarePartners has decades of experience helping people cope with a wide range of orthopedic problems and musculoskeletal conditions.

Our team of experts looks at each person’s circumstance and works closely with his or her orthopedic doctors to develop personalized treatment plans. Led by a physiatrist (a specialist in physical medicine and rehabilitation), your CarePartners team also includes physical therapists, occupational therapists, and nurses highly trained and experienced in orthopedic rehabilitation.

As an orthopedic patient, you might receive care at one of several CarePartners locations or in the comfort of your home.

  • Range of motion
  • Strength
  • Self-care ability, including activities of daily living (ADLs)
  • Home safety
  • Re-entering the community

  • Outpatient Rehabilitation – Five convenient locations with qualified therapists who are specially trained in orthopedic, sports medicine, aquatic and manual therapies
  • Home Health Therapy – Therapy and care at home, starting within 48 hours of your discharge from an acute care hospital and continuing based on your needs
